Dr. Pandeya provides Nephrology consultative services to patients in Oakville, Burlington, Milton, Georgetown and surrounding areas. He is a Fellow of the Royal College of Physicians of Canada, Fellow of the American College of Physicians, and an Assistant Adjunct Clinical Professor at McMaster University. He has Active Staff privileges at all three sites of Halton Healthcare, as well as Affiliate Staff privileges at Joseph Brant Hospital to provide a comprehensive patient care experience.

Consultation requests

Please fax consultation requests to Dr. Pandeya at 905-815-9492. You may wish to use the Ontario Renal Network (ORN) Referral Form and KidneyWise Toolkit where appropriate to help assist in making a referral for chronic kidney disease (see Resources section below).

The consultation request should contain the salient information including demographics, reason for consultation, and background history with appropriate appended information including but not restricted to the following:

  • Medication list
  • Relevant lab work including historical renal parameters
  • Ultrasound of the kidneys or other imaging studies
